Reporting Accidents

All accidents and injuries that occur on the HSU campus should be reported immediately. Call the HSU Police Department at 870-230-5098. You may be asked to complete an Accident Report Form to collect any information you have about the incident.

All work-related injuries and illnesses must be reported.

  • Call 911 immediately if an employee is seriously injured (loss of consciousness, loss of ability to communicate, chest pain, seizure, difficulty breathing, injury to the head and neck injury).
  • Inform the injured worker’s supervisor.
  • The injured worker’s supervisor (and the injured worker, if possible) should immediately call HSU Police Department at 870-230-5098.
  • The injured worker’s supervisor (and the injured worker, if possible) should immediately call the Injury Hotline at 1-855-339-1893.

If the employee’s injury is not serious, call the Injury Hotline before seeking medical treatment.

Employees should report even minor injuries to their supervisor prior to leaving work for the day.

In the event of a medical emergency (loss of consciousness, loss of ability to communicate, chest pain, seizure, difficulty breathing, injury to the head and neck injury) to any individual on campus, call 911 immediately. Then follow the guidance found in the “Medical Emergency” sections of this guidebook.
