Printable Entry Form for All Events 

• All teams please fill out the same entry form for all events desired.  Make sure you fill out the form in its entirety.  The sport you are participating in and the team captain information is VERY IMPORTANT.

 

• If you want to play but don' have a team, e-mail randleb@hsu.edu and get added to a free agent list with your name, e-mail address and your status(Undergrad/Grad/Faculty/Staff) so we can try and pair you with a team!
