Additional Information:
Lunch will be provided for one (1) representative from each organization registered for the fair. If additional lunches are required, please make sure to indicate on this form and fees are added accordingly ($10) to insure the appropriate amount is ordered.

Please notify us if you have a need for a large display area (unless otherwise requested, you will be provided one (1) six foot table adjacent to another company employer). Special arrangements must be made in advance by sending an email to or calling (870) 230-5442. We will make every effort to accommodate your needs.

Cancellation requests must be made in writing and received by Sheri Phillips no later than 5:00 p.m. Friday March 21, 2014. A $25.00 administrative fee will be retained for cancellations. No refunds will be made after Friday March 21, 2014. Please be advised that the company/school/vendor that is being represented will be responsible for payments (not the individual representative). 

Confirmations will be made via email (unless otherwise requested) upon receipt of completed registration form and payment. If you do not receive a confirmation, you should contact us to assure your information has been received. Otherwise, you should assume you are NOT registered.
