Young Alumni Achievement Nomination Form

The Young Alumni Achievement Award is presented to an alumni of Henderson State University who has graduated within the last ten years (eligibility is after 5 years from HSU graduation). Recipients must demonstrate significant accomplishment or promise in their career fields; leadership; or civic, cultural or charitable involvement.

Criteria for awarding the Young Alumni Achievement Award

  • One Young Alumni Achievement Award will be selected yearly and awarded during Homecoming.
  • Considerations include: career longevity, excellence in career, community or profession. 
  • Commitment to Henderson State University. 
  • Nominees cannot hold elected office.
  • Un-awarded nominations will remain as nominees for a 3-year period. 

Nominations may be submitted at any time of the year. Nominations should be submitted by June 30 to be considered for the current year. 

Nominators should use the first five fields for their own personal information. Remaining fields are for you to provide nominee information and qualifications.
